Job Description
Responsible for provision of case management services to homeless households in developing self-sufficiency skills and achieving housing stability.  Primary functions of the position include partnering with participants to:  assess strengths, needs and goals; develop an individualized service plan for meeting needs and achieving goals; refer and link to community resource providers; assess and implement services to build life skills; and evaluate follow-through and progress with meeting needs, achieving goals and program service requirements.

Program Operations
  • Support housing program operations consistent with funder requirements and agency program standards.
  • Collaborate with the Program Manager and Program Director to develop and evaluate program effectiveness, including service delivery; provide recommendations on revisions based on research and evidence-based methods.
  • Assess participants’ strengths, needs, and goals and develop correlating Housing Stability Plans.
  • Support and monitor participants in achieving action steps toward self-sufficiency goals identified in the Housing Stability Plan.
  • Provide supportive services as needed according to funding source guidelines and budget. 
  • Responsible for adhering to and reporting on program outcomes to the Program Manager and Program Director.
  • Aware of and strive to attain housing program performance measures per contract and/or initiated by the funder.
  • Responsible for complying with Agency and Department programs and policies.
  • Assist in the issuance of timely and comprehensive weekly, monthly and contract reports as assigned.
  • Ensure program delivery is supportive of the agency cultural competence and integration commitments including but not limited to disability, age, race, ethnicity, socioeconomic status, and sexual orientation.


Case Management Responsibilities
  • Guide participants through the referral and intake process, verify program eligibility and assist with completion of program required paperwork.
  • Maintain individualized files for each participant which documents all communication with program participants and funder required forms and paperwork.
  • Meet face-to-face with participants at a frequency as required by program funder through home or office visits.
  • Develop individualized plans based on participant strengths, needs and goals to address goals including housing, employment, education, training, life skills and financial literacy.
  • Support and monitor goals and progress on individualized plans.
  • Provide advocacy for participants, as appropriate, within the program and/or with external providers and make referrals to community services as needed.
  • Coordinate participant transportation to and from program service-related appointments or activities as resources allow and assist participants in developing skills to coordinate own transportation.
  • Complete required forms, paperwork and documentation associated with operation of program services.
  • Assist with the development and implementation of program group related activities.
  • Complete housing inspections and related paperwork as required.
  • Complete data entry of participant information, reviews, and case notes into Clarity/HMIS and CAP60 in a timely manner.
  • Monitor program unit conditions for safety, cleanliness, and maintenance during home visits.
  • Coordinate participant move-in/move-out activities.
  • Provide feedback and follow-up services for participants for a minimum of 6 months or based on funder expectations.
Internal/External Partnerships
  • Build and maintain working relationships with other service providers related to service delivery on behalf of Community Action programs.
  • Coordinate participant program services with other internal and external service providers.
  • Support document collection for funding proposals, funding source reporting and provide public speaking requests as needed.
  • Develop and maintain positive working relationships with funding sources, community partners, participants, and peers.
  • Consistently review strategy and objectives for ongoing program development in order to develop systems to improve program operations, coordination, and collaboration among staff.
  • Attend staff, department, agency, and local community meetings as required.

  • Follow-up on all program service referrals/requests as assigned.
  • Assist in maintenance and cleaning of Pathways facility.
  • Complete related work as assigned within the scope of responsibility and skills required for the job.

EDUCATION AND EXPERIENCE:
  • A Bachelor’s Degree in Social Work or a related field
  • Appropriate equivalent combination of education and work experience may also be accepted
  • Four years case management experience working with low-income individuals/families and poverty issues, including housing instability

LANGUAGE SKILLS: 
  • Requires excellent listening, verbal, and written communication skills, with strong attention to detail and accuracy.

MATHEMATICAL SKILLS: 
  • Knowledge of basic math: add, subtract, multiply, divide, fractions, and percentages. 

REASONING ABILITY:  
  • Ability to interpret a variety of instructions in written, oral or diagram form, to solve problems requiring decision-making skills within department guidelines. 

CERTIFICATES, LICENSES, REGISTRATIONS: 
  • Current social work certification from State of Wisconsin preferred.
  • Access to reliable, licensed, insured driver and transportation required.
  • Requires HQS certification

OTHER SKILLS AND ABILITIES: 
  • Must possess strong interpersonal, organizational, and independent work skills. 
  • Ability to work independently, as well as collaboratively, with diverse staff and participants and provide services in a structured, yet individualized, creative, and non-judgmental manner.  
  • Must possess experience with office equipment and procedures and basic budget and financial transaction procedures. 
  • Must have computer background and experience with Microsoft applications and internet.
  • Ability to proficiently utilize Management Information Systems, data entry.
  • Knowledge of local social service resources
  • Ability to maintain confidentiality of records/information and establish priorities.
