Social Worker - Maternal Health

UnityPoint Health - Allen

Shift: Full-time; 32 hrs/week

Sign-On Bonus Eligible: $7,500 paid over 2 years

As a member of the interdisciplinary team, provides social service support for pregnant patients/partners and adolescents/adults seeking reproductive and sexual healthcare. Works collaboratively with the Allen Women's Health Maternal Health (Iowa's Healthy Pregnancy Program/Title V) and Family Planning (Title X) teams to assist patients with addressing resources in the community, understanding the healthcare system, and identifying and addressing risk factors. This team member also provides support as needed to UnityPoint Clinics when appropriate.

Responsibilities
  • Functions as a member of the team to address risk factors and identify resources.
  • Performs psychosocial assessment of the patient to identify priority needs, strengths, patient preferences and barriers to care.
  • Provides intervention and support to patients/families to enhance their ability to cope with the impact of health conditions.
  • Assists with planning for care transitions and collaborates with UnityPoint Health, community services, and facilities to support patient safety and continuity of care.
  • Facilitates screenings as prescribed and required by the treatment team, Iowa Health and Human Services, and Title V/Title X programs.
  • Documents assessments, interventions, and referrals in the electronic health record and Iowa Health and Human Services database(s) according to documentation standards.
  • Complete application for presumptive Medicaid.
  • Serves as a patient/family advocate in support of patient confidentiality, informed consent, patient autonomy, and self-determination.
  • Assesses patient safety to identify possible abuse, neglect or other risks to safety and makes reports to state agencies as mandated.
  • Supports culturally competent services and assists with arranging interpreter services as needed.
  • Provides education to the patient/family regarding available services and supports and assists the patient to access those in which they are eligible.
  • Provides information and education to other team members in understanding the psychosocial implications of diagnosis and treatment options for the patient/family.
  • Participates in mentoring new employees and/or supervising social work interns as requested.
  • Provides community education and represents clinic at community events to promote the initiatives of the Title V and Title X programs.
  • Maintains comprehensive knowledge of community resources and acts as a liaison to refer patients/families to health and social services, health insurance, public assistance, and other resources to meet patient identified needs.
  • Provides expertise and plays a key role with the care team in establishing patient-centered goals of care and identifying psychosocial and behavioral strengths and barriers.
  • Contributes to the comprehensive, longitudinal plan of care based on patient-centric goals and coping strategies.
  • Collaborates with social workers, nurse care managers and other professionals across the continuum and in the community to ensure continuity of care.

Qualifications
  • Bachelor's degree in social work from an accredited school of social work.
  • One year of clinical experience in health care preferred.
  • Knowledge of the social work process
  • Awareness and sensitivity to cultural diversity
  • Knowledge of the physiological elements of illness and impact on psychosocial functioning
  • Knowledge of the healthcare system and resources available to patients.
  • Strong interpersonal skills and ability to work as a collaborative team member.
  • Knowledge of social determinants of Health
  • Strong verbal and written communication skills
