Description

LSS is currently seeking a Program Coordinator - Long-term Functional Screener for our Connections team in the northern part of the Wisconsin. This is a full-time opportunity that will function as the Program Coordinator and a Long-term Care Functional Screener.

Under the direction of an ICA Team Manager and LTCFS Liaison, the Program Coordinator performs professional work involving the coordination of Long Term Care Functional Screen (LTCFS) team lead responsibilities, IRIS consultants and other related agencies in order to establish and maintain high quality screening services.


ESSENTIAL DUTIES AND RESPONSIBILITIES:
This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.

  • Fulfill duties of the Department of Health Services (DHS) lead screener as outlined in the job description/addendum.
  • Provide new screeners with materials and guidance necessary to support successful completion of the online functional screen training course.
  • Ensure additional training, mentoring, and monitoring for new and experienced screeners occurs in accordance with the established program's LTCFS training plan.
  • Act as a contact with whom other IRIS Consultant Agencies (ICA) or Managed Care Organizations (MCO) can contact upon need of a LTCFS transfer.
  • Consult with MCO or IRIS screeners when a person is found functionally eligible by the Aging and Disability Resource Center's (ADRC) initial screen, but the MCO or ICA finds them ineligible or eligible at a non-nursing home level of care.
  • Review and determine outcomes for requests for cross validations from Self Directed Personal Care (SDPC) and Medical Assistance Personal Care (MAPC) Registered Nurses.
  • Review and determine outcomes for requests for Change of Condition rescreens.
  • Participate in LTCFS meetings and webcasts offered by DHS.
  • Consult with DHS on specific participant situations related to screening accuracy or interpretation.
  • Update program procedures as needed.
  • Coordinate training and mentoring for new screeners.
  • Coordinate the quality review process of program's LTCFS team.
  • Share results of reviews with screeners, Program Manager and ICA director to identify issues, trends and training needs.
  • Under the direction of the Screen Liaison, work with DHS and colleagues (screeners, Program Manager, and ICA Director) to address quality concerns or remediation as needed.

QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE:

  • A Bachelor's Degree in social work, psychology, human services, counseling, nursing, special education or a related field or an equivalent combination of education and/or experience is required.
  • A Minimum of one year of experience completing LTCFS with seniors and/or people living with disabilities preferred.
  • Has and is able to maintain a valid Certification of a Long Term Care Functional Screen.
  • Have a strong commitment to enabling people to be in control of their lives.

  • Capacity to support individuals served to meet required IRIS policies and procedures in ways that maximize administrative ease.

  • Ability to establish and maintain effective public and working relationships.



CERTIFICATES, LICENSES, REGISTRATIONS:
Must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.

TRAVEL: Ability to travel on day trips as required up to 50%. Some overnight travel may be required.
