Job Description

Work Schedule:

This is a full-time, 100% FTE evening/night position. Schedule will rotate every two weeks, working 4 10-hour days. Hours may vary based on the operational needs of the department.

Week 1: Tuesday, Wednesday, Friday and Saturday 2:30pm - 12:30am

Week 2: Sunday, Wednesday, Thursday and Friday 2:30pm - 12:30pm

We are seeking a Social Worker to:

  • Act as communication liaison between medical team and patient's family/support system during a medical emergency
  • Provide support for patients and families experiencing acute grief response during a medical emergency.
  • Provide general community resources for patients who are experiencing psychosocial issues which may impact their physical and emotional health, and/or their ability to care for themselves.
  • Review and create patient advance directives.

Education:

Minimum -

  • Master's degree in Social Work

Work Experience:

Preferred -

  • Two (2) years prior hospital based social work experience, behavioral health background.

Licenses and Certifications:

Minimum - Licensed in the state of Wisconsin with one of the following within 90 days of hire:

  • Certified Advanced Practice Social Worker (CAPSW)
  • Certified Independent Social Worker (CISW)
  • Licensed Clinical Social Worker (LCSW)

Required Skills, Knowledge, and Abilities:

  • Assessment and evaluation skills
  • Ability to work independently, while collaborating effectively as a team member with other health professional team members
  • Proven skills in identification of stressors affecting patients and their families.
  • Knowledge of and ability to work within a complex health care system.
  • Expertise in conflict mediation, abuse and neglect, advance directives, crisis intervention, government programs

Physical Requirements:

Sedentary: Ability to lift up to 10 pounds maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools. Although a sedentary job is defined as one, which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary if walking and standing are required only occasionally, and other sedentary criteria are met.
