The Social Worker Patient Advocate supports quality care outcomes by responding to patient and family concerns. The position expedites the best outcomes for patient and families by utilizing a collaborative, multi-disciplinary approach focusing on the patient and family centered care model. The Social Worker Patient Advocate utilizes their clinical knowledge to aid in advocacy and collaboration with the interdisciplinary care team.

The Social Worker Patient Advocate takes, reviews, and assesses the complaints and grievances and inputs the information in the RLDatix software. Communicates patient concerns to the appropriate staff to provide the best outcome for the patient and UnityPoint Health - Meriter.

Coordinates with Risk Management, Patient Safety, Corporate Compliance and Legal Counsel to provide positive patient outcomes, and to mitigate complex risk and legal factors. Utilizes their clinical knowledge and experience to identify compliance, risk and safety issues. Coordinates with UW Patient Relations on patient complaints and grievances that cross health systems.

The Social Worker Patient Advocate works collaboratively and communicates effectively with a variety of disciplines to include the direct care team members, managers, directors, providers and executive leadership. Is able to communicate clearly and effectively both verbally and in writing.

Is fluent in using on-line improvement tools and administrator of system Patient surveys. Provides leadership, and role models behaviors and tactics to improve patient experience. Presents to committees as needed.

Required Qualifications:
  • Master's Degree in Social Work.
  • 3-5 years Healthcare clinical experience working with patient and families.
  • Licensed Social Worker in the state of Wisconsin or eligible within 1 year of hire.
  • Client Rights Specialist Training to be obtained within 6 months of hire.
  • Proficient computer skills relevant for the role.
  • Excellent verbal and written communication.
  • Process improvement experience. Ability to analyze current processes and patient/family interactions, identify opportunities for improvements and make recommendations.
  • Ability to act independently while maintaining an environment of teamwork and collaboration.
