Job Description
Overview

The social worker assesses and assists with creating a plan to meet the mental health needs of patients. They work within the multi-disciplinary team to provide patients with a safe environment; education and knowledge to improve coping skills, support networks and outpatient follow up.
Responsibilities

Essential Duties and Responsibilities

  • Works in close collaboration with physicians, nurses, occupational therapist/recreational therapist, patients and families to evaluate patient needs and understand significant social and emotional factors that underline patient's current mental health problems.
  • Assesses and reassesses for continuing treatment needs, prepares patient histories, discharges and contributes to the plan of care.
  • Reviews plan of care with patient (family as indicted) as required.
  • Assess patient needs and develops appropriate dispositional plans and coordinates care with the appropriate departments.
  • Maintains strong communication with members of treatment team.
  • Facilitates education groups and individual sessions to address treatment plan goals with patients (and families as appropriate).
  • Utilizes verbal de-escalation skills when dealing with agitated patients.
  • Assists with admission and discharge: paperwork, belongings, patient and room searches, 15 minute monitoring of patients
  • Complete ED and medical floor consults.

Education and Experience

Bachelor's degree in health and/or human services field


Certification and Licensure

Licensed as a CSW or LSW (if working in the state of Wisconsin).
Approved CPR or BLS certification required within 90 days
Verbal de-escalation, self-defense and control techniques training within 90 days.

Special Physical Demands

While performing the duties of this job, the employee is frequently required to sit, talk and hear. The employee is occasionally required to stand, kneel, walk and on occasion run short distances; use hands to handle, finger and feel; and reach. The employee must exert up to 50 pounds of force occasionally. Specific vision abilities required by this job include frequent close vision. Occasional distance vision and ability to adjust focus.
